The Peter T. Simonian, M.D. and Patricia L. Simonian Family Foundation is a nonprofit private foundation organized and operating under the laws of the State of California. The Foundation is recognized by the Internal Revenue Service as a tax-exempt organization under Section 501(c)(3) of the Internal Revenue Code, with an Employer Identification Number (EIN) of 47-2573861. Its principal office is located at 729 N Medical Center Drive West, Suite 101, Clovis, California 93611-6880. Established in March 2015, the Foundation operates as an independent private grantmaking entity, with contributions being tax-deductible to the extent permitted by law. The Foundation is governed in accordance with its bylaws and charter, and is committed to supporting charitable, educational, and healthcare-related initiatives in line with its philanthropic mission
